Adventist Health Ukiah Valley and a group of nonprofit organization partners are conducting a survey of community health and well-being. Completing the short anonymous survey will provide information to the Adventist Health Ukiah Valley’s Community Health Needs Assessment. The survey is open through August 23, and can be found at adventisthealthchna.org/mendocino.
